Purpose:
|
To report a case of breast cancer with metastasis to orbital apex with manifestation of supradivisional third nerve palsy.

: A 40 year old female known case of breast cancer since 3 years ago referred to our Nuero ophthalmologic clinic with chief complaint of ptosis and diplopia. In ophthalmic examination, visual acuity of both eye were 20/20 and there were no significant finding in anterior and posterior segment ophthalmic examination. There was left eye ptosis with MRD1 of 2mm. In ocular movement examination, there was limitation of motion in supra duction (-2 to -3) of left eye. Orbital MRI with contrast was requested for patient and demonstrated a lesion in orbital apex area of left eye that showed enhancement after contrast injection. At the end, the patient was diagnosed as supradivisional third nerve palsy and orbital radiotherapy was recommended for the patient.

Conclusion:
|
In a patient with breast cancer presenting with diplopia, metastasis to orbital apex with manifestation of divisional third nerve palsy should be kept in mind and appropriate orbital imaging should be considered. To the best of our knowledge metastasis of breast cancer to orbital apex with presentation of divisional third nerve palsy have not been reported.
