Purpose:
Grave’s disease is an autoimmune disease .Increasing of intraocular pressure in up gaze position can be observed in all patients with proptosis and in %68 of patients without proptosis. Intra ocular hypertension is not preventable, but glaucoma and optic neuropathy can be prevented with early diagnosis and careful follow up
Methods:
94 patients with Grave’s orbitopathy were referred to Farshchian hospital for complete ocular examinations. Primary informed consent obtained from all subjects, and intraocular pressure measured by Goldman tonometer. Disease classification performed by NOSPECS system
Results:
Sixty-seven patients (%72.05) were female and 27 patients (27.95) were male. Majority of patients had a disease period between 0-24 month and most of them were in stage 3 of disease. Nine patients (9.6%) had IOP ≥21mmHg and mean of IOP was 16.61 ± 3.42. mean of IOP in males and smoker patients was significantly higher than females and non-smokers. Also, IOP was significantly increased with increasing in disease stage, disease duration and patients age. In this study, 7.52% of patients were diagnosed with grave’s ophtalmopathy and antithyroid drugs were consumed before entering the study and not new cases.
There is a significant difference in IOP Mean score between patients who consumed drug (old cases) and patients who not consumed drug (new cases). limited eye movements was significantly higher in males and smoker patients in comparison to females and non-smokers.
Conclusion:
According to our finding, majority of patients were female but prevalence of eye manifestations in male was higher than female. Higher IOP was related to old cases (22.85 mmhg) ,smoker , male gender and patients with limitation of extra ocular movement.. Most common ophthalmic signs in patients were eyelid retraction and preorbital edema. Prevalence of intraocular hypertension was higher with compare to other study.
