Purpose:
To describe a rare case of sphenoid sinus mucocele presenting as retrobulbar neuritis and to highlight the role of imaging in diagnosis and treatment planning.
Methods:
A 27-year-old woman noted a subacute decrease in vision in her right eye which was compatible with optic neuritis. Magnetic resonance imaging (MRI) showed a large sphenoid mass compatible with sphenoid sinus mucocele.
Results:
The mucocele was removed by an ENT specialist and the diagnosis was confirmed by pathology; the patient was also treated with high dose corticosteroid, and then progression of the disease was stopped. Unfortunately, no significant improvement in the vision was achieved.
Conclusion:
This report emphasizes on the important role of imaging for distinguishing between different causes of optic neuritis. A high index of clinical suspicion along with appropriate imaging studies can help us diagnose rare causes. With timely management, gratifying results may be obtained.
