Purpose:
|
Spherophakia is a congenital bilateral anomaly of the lens in which the crystalline lens is small and spherical due to zonular developmental abnormality. Clear lens extraction seems to be the first choice to treat associated lenticular myopia and control glaucoma. In the current study we aim to evaluate the efficacy of lensectomy, anterior vitrectomy and artisan implantation in patient with spherophakia induced glaucoma.

Methods:

patients with spherophakia induced glaucoma who had undergone lensectomy, anterior vitrectomy and Artisan implantation during 1385-1395 was studied.

Results:
|
Mean of logMAR CDVA was improved from 0.92 ± 0.57 before surgery to 0.17 ± 0.15 at the last follow-up after surgery (P = 0.001).Based on this analysis, we expected 1.788 mm Hg decrease in IOP at 1 year after surgery. However, after 4 years we observed a decrease in the decrement of IOP.

Conclusion:
|
Patients with spherophakia have decreased visual acuity mainly due to glaucoma and high refractive errors. Affected individuals are mainly young. So it is important to early detect and treat these patients to prevent blindness. There is no definite guideline for treatment of glaucoma associated with spherophakia. In some patients it is necessary to extract the lens which also alleviates the high myopia. Due to small capsular bag and lack of sufficient support of CTR in severe zonular instability encountered in patients with spherophakia, sclera fixation IOL or ACIOLs may be preferred; as in our cases patients had improved visual acuity and better IOP control after lensectomy anterior vitrectomy and artisan implantation and it can be an effective choice in these patients.
