Purpose:

To report a patient with isotretinoin embryopathy presented with monocular elevation deficiency (MED), bilateral external auditory canal atresia and hearing loss since birth

A 4 years old girl was referred to our clinic with chief complaint of inability to elevate the right eye since birth. She has been a preterm infant of 35 weeks' gestation who was born to a 36-year-old Mother.The mother's medical history was significant for facial cystic acne treated with isotretinoin 20mg/day, for the first 3 months of gestation.The pregnancy was uncomplicated and in prenatal ultrasound examination no fetal abnormality was detected.Physical examination revealed bilateral external auditory canal atresia and patient suffered from hearing loss.Extraocular movement examination showed absent elevation of the right eye in primary position and right and left gaze.Forced duction testing under general anesthesia showed marked restriction to supraduction of the right eye. She underwent 5.5mm recession of right inferior rectus muscle. Postoperatively, she was orthotropic in primary position with improvement of supraduction limitation of the eye.

Conclusion:

Monocular elevation deficiency (MED) may occur as a presentation of isotretinoin embryopathy.In our patient bilateral external auditory canal atresia and hearing loss were other presentations. To our knowledge, this is the first report of MED as a manifestation of isotretinoin embryopathy
