Purpose:
To report a patient with bilateral optic disc edema due to lead poisoning.
Methods:
The patient was a 42-year-old male who, due to lead poisoning, had been treated with lead chelators. He had gradually developed blurred vision in both eyes over 40 days.
Results:
He had ataxia and paresthesia and a toxic level of serum lead. His visual acuity in both eyes and his color vision deteriorated. Humphrey’s Visual Fields test revealed a double arcuate scotoma with enlargement of the blind spot. In funduscopy he was found to have bilateral optic disc edema which was confirmed by optical coherence tomography and fluorescein angiography. Brain MRI results were normal. This is the first reported case with these specific impairments.
Conclusion:
In patients with optic disc edema, taking a complete history has a key role in diagnosis of its cause. In cases of chronic usage of oral opium, lead toxic effects should be kept in mind, because it is sometimes added to opium products by profiteers.
